Geriatric Rehabilitation Program

Objective
The purpose of the Geriatric Rehabilitation program is to evaluate and treat clients who have experienced a loss of autonomy as a consequence of a period of immobility or illness. These clients require an evaluation of their potential for rehabilitation and reintegration into the community, as well as the interventions of a variety of health care professionals in order that they may regain their autonomy.

Approach
The Geriatric Rehabilitation Program is a small, specialized rehabilitation team that treats and records each client's progress. The intensive focus and collaboration of the team members enables us to offer rehabilitative care to clients.

The Geriatric Rehabilitation Program is designed to address the specific goals of each client. One key objective of the program is to develop an appropriate discharge plan for each client that will promote health, safety and quality of life. Through an ongoing and intensive process of consultation with the client and their family, a discharge plan, which can best meet their needs and goals, is implemented.

Consistent with the philosophy of the Jewish Rehabilitation Hospital, the Geriatric Program utilizes a multidisciplinary team. Team members may include:
  • Dietician
  • Nurse
  • Occupational therapist
  • Pharmacist
  • Physician
  • Physiotherapist
  • Psychologist
  • Rabbi and/or priest
  • Recreation
  • Social worker

  • Clientele
    The clientele of the Geriatric Rehabilitation Program are elderly (the majority of the clients are over 80 years of age) and all have experienced a loss of autonomy. Many of the clients are admitted following a major surgical procedure or illness.

    Admission Criteria
    It is important that the client's medical status be stabilized at the time they begin their intensive rehabilitation program. To be admitted to the program, clients must have a referral from a healthcare professional at their hospital, their CLSC, or from another geriatric service. Potential clients must demonstrate:
    1. Realistic potential for improvement in activities of daily living or instrumental activities of daily living(as determined by an OT or PT).
    2. Realistic potential for community reintegration at the level of home-care, residential or foster home level of care (as determined by a social worker).
    3. Requiring active interventions by at least one rehabilitation discipline while in acute care.**

    **Clients admitted under the program for Geriatric Rehabilitation Program require assessment and/or interventions by rehabilitation nurses, medical care professionals, physiotherapists, OT's, and social workers.

    Referring professionals should submit an admission request and/or referral form to the JRH Admissions Office.
