Non-Institutional Resources in Physical Disability Program

MISSION
Created in November 2004, the residential program fills a regional mandate of non-institutional residential resources in physical disability. It encompasses two aspects, the first of which includes recruitment, evaluation of applicants and resource follow-up. The second aspect consists of ensuring admission and follow-up of the users housed within the program resources. This program aims to offer a diversity of living environments according to the housing needs of the clientele and their families in order to maintain or promote optimal social participation.
Aspect: “Recruitment and evaluation of applicants”:
Goals:
- Recruit and select applicants who offer a physical environment as well as life hygiene and safety conditions compliant with program standards, while ensuring respect of the interests and rights of the users;
- Support resources and ensure services and resources quality.
Offer of Services:
- In collaboration with the program coordinator, a social worker oversees recruiting, admitting and evaluation of applicants. She also ensures resource follow-up and re-evaluation as well as the application of their agreement.
Clientele:
- Any adult or organization in compliance with the program's general selection criteria interested in offering residential housing services as well as support and intervention for users within the program. All resources already accredited.
General Eligibility Criteria for Applicants:
- Adult physically and mentally fit, with a stable family environment since at least two years and the approval of all family members for their resource project; have a healthy and balanced financial situation; offer a physical environment in compliance with program standards and user needs.
Aspect: “Admission and follow-up of users”
Goals:
- Allow the user to adapt and remain as long as possible in his/her substitute living environment;
- Maintain or support the gradual return to regular lifestyle and social roles linked to socio-residential, socio-community, school and occupation autonomy;
- Maintain or increase the social participation or social re-insertion within community activities.
Offer of Services:
- The program's multidisciplinary team, comprised of a coordinator, an occupational therapist, a specialized educator, a social worker, a psychologist and a secretary, offer bilingual services, mainly within the substitute environment of the user. In addition to responding to the housing need, the interventions aim to reduce handicap situations and encourage optimal social participation of the user along with the involvement of the resource, the family and various partners.
Clientele:
- Any person younger than 65 years of age, residing in the Laval region, presenting significant and persistent physical disabilities resulting mainly from a physical disability (auditory, motor, speech, visual) and who requires permanent non-institutional housing due to the impact of those disabilities on the carrying out of his/her autonomous lifestyle habits.
Eligibility Criteria for the Users
- Any Laval resident referred by a CSSS representative and complying with the definition of the program clientele and for who home support is no longer possible. The physical and mental health state of the referred user is stable and does not require medical and nursing care on a continuous basis. The services required by his/her condition can be provided in a family or a non-institutional living environment.
Research, teaching and continuous education
- The caregivers within the non-institutional resources program constantly take part in continuous education activities.
